A Checklist for Vaccinations in Newborns

3.            3-4 months- Diphtheria, tetanus and whooping cough (pertussis) (DTaP) (dose 2) Haemophilus influenzae type B disease (Hib) (dose 2), Polio (IPV) (2nd dose), Rotavirus (RV) (2nd dose)



4.            5-6 months - Diphtheria, tetanus, and whooping cough (pertussis) (DTaP) (dose 3), Haemophilus influenzae type b disease (Hib) (dose 3), Polio (IPV) (dose 3), Rotavirus (RV) (dose 3)



5.                  7-11 months- Influenzae (flu)

6.                  12- 23 months - Chickenpox (Varicella) (dose 1), Diphtheria, tetanus, and whooping cough (pertussis) (DTaP) (dose 4), Haemophilus influenzae type b disease (Hib) (dose 4), Measles, mumps, and rubella (MMR) (dose 1), Polio (IPV) (dose 3), Hepatitis A (HepA) (dose 1), Hepatitis B (HepB) (dose 3 between 6 months and 18 months
